Uterosacral nerve ablation (USNA) is a specialized surgical process aimed at alleviating chronic pain. Especially for those women who are suffering from endometriosis or other gynecological conditions. In the medical field, USNA is also known as uterosacral ligament ablation and laparoscopic uterosacral nerve ablation (LUNA). This procedure involves cutting or ablating the nerves that connect the uterus to the brain, which can help to relieve pain. Pain signals are sent from the pelvic organs to the brain via the uterosacral nerves, which are a component of the autonomic nervous system.

Through the destruction or removal of a portion of the uterosacral nerves, these USNAs are likely to interact with these pain signals. The specific case and the doctor’s preference will determine whether this activity is carried out openly or laparoscopically. The success of the operation in terms of symptom relief can differ. Some women may experience significant improvement in their symptoms after the USNA surgical procedure.

Types of Uterosacral Nerve Ablation

There are two types of USNA surgery and they are listed below:

1. Laparoscopic uterosacral nerve ablation (LUNA)

  • This is most often referred to entail laparoscopic surgery. It cuts or destroys a small portion of the uterosacral ligament. This is responsible for carrying nerve fibers within the pelvis. Laser or electrodiathermy techniques can be used to perform the ablation.

2. Open surgery

  • After locating the uterosacral ligaments, the nerve fibers inside are carefully dissected, transected, or ablated with scissors, electrocautery, or a laser. The conventional method for performing USNA is open surgery, sometimes referred to as laparotomy.

Uterosacral Nerve Ablation Causes

This process is used to treat CPP in females. It is common in the ones who have not responded to other treatments. The procedure is usually meant for women with:

Chronic pelvic pain

  • It is defined as recurrent or persistent pelvic or lower abdominal pain that lasts longer than six months and is treated with USNA.

Endometriosis

  • USNA works well for treating CPP linked to endometriosis, especially when the condition is mild to moderate. 

 Dysmenorrhea

  • Painful sexual relations and painful menstruation are both treated with USNA.

Pelvic pain

  • This can be effectively treated by USNA, which targets the uterine sympathetic innervation at the level of the superior hypogastric plexus.

Gynecologic issues

  • A number of underlying conditions inclusive of gastrointestinal, urologic, and musculoskeletal issues. They can contribute to chronic pelvic pain.

No underlying pathology

  • In certain situations, when there is no discernible underlying pathology. USNA may be utilized as a surgical denervation technique to block pain pathways.

  • Persistent pelvic pain

How is Uterosacral Nerve Ablation Surgery Performed?

  • General anesthesia drug is usually used during the surgery.
  • To view the pelvis, the surgeon will insert a laparoscope through small stomach cuts.
  • After determining the location of the uterosacral ligaments. These nerve fibers are cut with a laser electrodiathermy razor or harmonic scalpel, among other techniques.
  • The aim of the operation is to destroy the sensory nerve fibers and secondary ganglia. These tissues carry pain from the uterus and other pelvic structures.
  • Once the process is complete, the surgical cuts will be closed. This will be done with the help of stitches. It totally depends on the surgical technique used.

After The Procedure

  • The whole process takes around one to two hours.
  • Patients will be observed in the recovery room.
  • Patients can be sent home from the hospital after one to two days.
