Description

Pelvic adhesiolysis is a process of surgery in which adhesions in the pelvis are either removed or dissolved. Such problems occur when tissues in the pelvis stick together abnormally. Binding of the scar tissue can cause pain, discomfort, and complications during fertility in women. Growth of abnormal tissue can develop anywhere in the pelvic cavity. This might vary in size, stiffness, and composition.

Pelvic adhesions may affect other body parts in or around the surface of the pelvic. It may affect various areas, such as the uterus, ovaries, fallopian tubes, or bladder. The main goal of the adhesiolysis is to relieve symptoms, such as pelvic pain and infertility, and to improve the overall quality of life for patients with adhesions. Advanced minimally invasive surgery is being used more and more to treat this condition. Because it involves smaller incisions, which reduces pain, post-operative complications, and the risk of recurrent adhesions.

Types of Pelvic Adhesiolysis Treatment

There are multiple types of this treatment. These are listed below:

1. Laparoscopic

  • This is also known as keyhole surgery. In this process, a surgeon makes a small cut in the stomach with the use of a laparoscope and bonding of tissue is removed.

2. Open surgery

  • In this case, a single cut is made through the midline of the body.

3. Hysteroscopic

  • A surgery that removes bonding of tissue from inside the uterus.

4. Epidural

  • A process that destroys stiff fibrotic material bonds that can cause nerve irritation during spine surgery.

5. Peritoneal

  • An operation that destroys fixing between various organs and the inner layer of the abdominal wall.

6. Adnexal

  • These growths are close to the uterus or ovaries that can be surgically removed.

Causes of Pelvic Adhesions

a) Infection

  • Pelvic inflammatory disease caused by STIs.
  • Tuberculosis from the patients who are living in the nearby area.
  • ill health conditions, such as appendicitis or peritonitis, which can lead to adhesion formation.

b) Surgical

  • Incomplete excision of endometriosis.
  • C-section surgery.
  • Removal of ovarian cysts.

c) Chronic condition

  • Polycystic ovary disorder.
  • Irritable bowel syndrome or inflammatory bowel disease.
  • Endometriosis.

d) Other causes

  • Radiation therapy for cancer.
  • Peritoneal dialysis for kidney failure.
  • Incomplete healing or inflammation after surgery.

Symptoms of Pelvic Adhesions 

This can cause a range of indications which depends on the severity of the condition. Some of the common features related to this ill health condition are listed below:

  • Pain in the stomach or pelvis area.
  • This could lead to hormonal imbalances.
  • Irregular menstrual periods.
  • Bowel obstruction or compression.
  • Pain during sexual intercourse.
  • Infertility.
  • Difficulty in getting pregnant.

Procedure of Pelvic Adhesiolysis

Before the Process

Medical evaluation
  • To determine any possible dangers or consequences and to evaluate your general health. For this, a comprehensive medical evaluation will be carried out.

Preoperative instructions

  • Before the activity, you might be told to refrain from eating or drinking for a certain amount of time. 

Consent and conversation

  • The patient can talk about the procedure’s advantages, possible drawbacks, and alternate forms of treatment. The procedure will be performed with consent.

Preparation

  • You will be asked to wear a hospital gown.
  • The medical staff will start an IV in your arm or wrist. For giving medicine or fluids during the surgery.
  • This surgery works under general anesthesia. It ensures that the patient remains asleep and does not feel any pain during the procedure.

During the Procedure

  • The doctor will make small cuts on your stomach.
  • The patient will administer gas to inflate the stomach and the doctor can look inside clearly. 
  • From the one cut, the surgeon will insert a thin, long tube with a camera. With the help of this, a specialist can see an internal organ on a screen.
  • Once the adhesion is located, then the surgeon will insert a small instrument through the other cut.
  • And then the doctor will cut out the adhesion from the affected organ.
  • At last, the surgeon will remove all the instruments from the cuts and close the incisions with adhesive strips or stitches.

After the Procedure

  • The operation takes around one to three hours.
  • Then the patient will be shifted to the recovery room.
  • Once the patient feels better, he or she can start walking.
  • To treat postoperative discomfort, your doctor will prescribe the appropriate painkillers.

Precautions To Be Taken After the Surgery

  • Try to keep the operated area covered with proper bandages.
  • Patients are advised to take their routine medications.
  • A fiber rich diet and plenty of water should be taken.
  • People should avoid carrying heavy weights.
  • Twice daily, apply ice for fifteen to twenty minutes. The patient will no longer experience recurrent discomfort as a result.
